About Halton-Peel Community Aphasia Programs

Halton-Peel Community Aphasia Programs are social communication groups for people living with acquired communication disorders.  Our programs involve both small and large group activities and are intended for people who want to improve their communication.  As an organization, we educate the community about aphasia and provide training and support to family members, friends, and caregivers.

Our goal at H-PCAP is to enhance the communication skills and quality of life for people with aphasia using a Life Participation Approach to Aphasia (LPAA).  Our services take into account the clients’ perspective and current communication abilities, and our programming is designed to reflect the clients’ varying needs, wants, and abilities.
 

The program strives to enhance the life participation of its members who are affected by aphasia and other acquired communication disorders by:
  • Improving communication skills
  • Providing access to information in aphasia-friendly formats
  • Supporting caregivers
  • Spreading community awareness of aphasia 
  • Reaching out to those with aphasia; reducing isolation and depression

H-PCAP services are based on best practice for working with adults with aphasia.  Together with support of the SLP, each client selects a functional Life Participation Goal to target within the group therapy sessions.  These goals are intended to enhance communication skills and participation in activities in and out of the therapy room, and ultimately improve a client’s quality of life. 

Group Therapy serves multiple purposes:
  • Provides emotional support
  • Connects people with others going through similar experiences
  • Socialization
  • Conversation opportunities
  • Builds on skills learned in individual therapy sessions

Who We Are

H-PCAP group therapy sessions are facilitated by a Speech-Language Pathologist (SLP) and Communicative Disorders Assistants (CDAs) contracted through Monarch House Oakville, a division of CBI Health Group. Together, H-PCAP and Monarch House continue to advocate for people with aphasia and are working to expand aphasia awareness and services in Southern Ontario.  Our community-based conversation groups  are delivered within local communities, such as Burlington, Oakville, Milton, Mississauga and Brampton.

Our program utilizes resources including funding, supplies and equipment in an efficient and non-wasteful manner.  Resources, ideas and information are shared among program sites and other community partners.

Our History

Halton-Peel Community Aphasia Programs began in 1984, when a group of stroke survivors met at St. Christopher’s Anglican Church in Burlington for lunch and socialization.  The group has gone through several name changes, from the Fellowship of Stroke Survivors to the Halton Aphasia Centre, and continues to evolve to meet the needs and best practices of those living with aphasia. In March 2014, the Central West LHIN approved funding to expand our program into the Peel Region.  

Now called the Halton-Peel Community Aphasia Programs, our clinical team facilitates fourteen communication groups out of Burlington, Milton, Oakville, Mississauga and Brampton. With over 130 members among all groups, our program continues to grow and expand, which highlights the need for aphasia programs in the community.
