The clinical and professional services for H-PCAP are provided through a contract with Monarch House, a division of CBI Health Group. Together, H-PCAP and Monarch House continue to advocate for people with communication disorders and are working to expand aphasia awareness and services in Southern Ontario, Canada.

The Role of the Registered Social Worker (RSW):
- Provides clinical assessments for clients
- Arranges new client intakes and performs “Meet and Greets”
- Assists clients in developing individual communication goals
- Facilitates large and small group language-based activities
- Provides aphasia training for clients/families, volunteers & community members
The Role of the Communicative Disorders Assistant (CDA):
- Facilitates large and small group language-based activities
- Facilitates small group conversations
- Help clients work on their individual communication goals
- Plans weekly activities with guidance from the SLP
The Role of the Registered Social Worker (RSW):
- Meets with clients and/or caregivers 1:1 to provide support
- Provides caregiver support groups
- Helps facilitate group conversations
